Bepotastine, chemically known as (+)-(S)-4-{4-[(4-chlorophenyl)(2-pyridyl)methoxy]piperidine}butanoic acid benzenesulfonate, is a non-sedating, selective histamine H1 receptor antagonist. It is primarily used for the treatment of allergic rhinitis, urticaria, and itching caused by skin diseases such as eczema, dermatitis, prurigo, and pruritus. Bepotastine exhibits additional antiallergic activities by stabilizing mast cell function, inhibiting eosinophil infiltration, suppressing IL-5 production, and inhibiting leukotriene B4 (LTB4) and LTD4 activities. It also has the effect of inhibiting the expression of nerve growth factor (NGF), which may be one of the mechanisms by which it works in treating allergic diseases. The onset of action for bepotastine is 0.25 hours, with a duration of 12-24 hours. In clinical trials, bepotastine has shown significant inhibition of IL-5 production and a reduction in histamine release in vitro. Additionally, bepotastine does not interact with serotonin, muscarine, benzodiazepine, and β-adrenergic receptors, thus avoiding adverse reactions such as dry mouth or drowsiness. Bepotastine is available in oral form in Japan under the trade name Talion and has been approved in the United States as an ophthalmic solution under the trade name Bepreve.
